A 37yrs old female presented to casualty with fever, headache, and altered sensorium of 6 days duration. an episode of seizure At admission, She was Afebrile with a pulse rate of 110/min and a blood pressure recording of 100/70mmHg; her Glasgow Coma Scale score was 10 (E3M2V5). There were no petechiae or signs of bleeding over the skin or any mucosal surface. There was no focal neurological deficit and neurological examination was normal, including (absence of) signs of meningeal irritation. patient is on infusion Noradrenaline and vasopressin support.Kindly discuss the case and give your expert advice….

Dr Dharm Kumawat Jaisalmer *Chief Complaints* fatal congenital disorder (abnormal formation of the vertebral curvature , Exposure of the abdominal organs ) making it difficult for the fetus to be born normally *History* Goat with first kidding suffering from dystocia since 1 night [ Dystocia due to Schistosomus reflexus (SR) ] Schistosomus reflexus (SR) - Schistosomus reflexus (SR) is a rare and fatal congenital disorder that can affect fetuses in goats, and is characterized by: Abdominal organs: Exposure of the abdominal organs Spine: Spinal inversion and marked ventral curvature Limbs: Limb ankylosis and positioning of the limbs close to the skull Other: Hypoplasia of the lungs and diaphragm, scoliosis, cleft sternum, and abnormalities of the digestive and urogenital systems SR can be caused by genetic factors, mutations, chromosomal anomalies, infectious agents, environmental factors, or a combination of these factors. The structural deformities caused by SR can lead to dystocia, making it difficult for the fetus to be born normally. In most cases, the fetus needs to be removed by cesarean section. *Management* C Section / Laprohysterotomy
